The deaths have not been from the cap.  Anyone who is unruly enough to
receive the cap generally also gets 'hobbled' which is a method of
restraining people in a contorted position.  This results in a phenomena
known as 'positional asphyxia'.  Certainly gasping for breath from the
cap intensifies the difficulties that result from this position but the
cap itself does not cause this.
